Yes No Acknowledgement of Risks * I, the undersigned parent or legal guardian of the above-named participant, acknowledge that participation in swim lessons involves inherent risks, including but not limited to the risk of drowning, injury from slips or falls, and exposure to environmental elements. While every effort will be made to ensure the safety of participants, I understand that such risks cannot be entirely eliminated. Agree Disagree Release of Liability * In consideration of my child’s participation in swim lessons, I hereby release and hold harmless The Neighborhood Mermaid, its employees, agents, and representatives, from any and all liability, claims, or causes of action that may arise from injury, illness, or damage sustained by my child during participation in swim lessons, except where caused by gross negligence or intentional misconduct. Agree Disagree Medical Treatment Authorization * In the event of an emergency, I authorize The Neighborhood Mermaid to secure medical treatment for my child if deemed necessary. I agree to assume financial responsibility for any medical expenses incurred as a result of such treatment. Agree Disagree Food Intake Policy * For the safety and comfort of all participants, I acknowledge and agree that my child must not consume any food or heavy snacks within 30 minutes prior to the start of their swim lesson. I understand that swimming on a full stomach can increase the risk of discomfort, vomiting, and other medical issues during aquatic activities. I further understand that failure to comply with this policy may result in the suspension or cancellation of the lesson at the instructor’s discretion, without refund. I agree to inform any other guardians or caregivers responsible for bringing my child to lessons of this policy and ensure their compliance. By signing below, I acknowledge that I have read, understand, and agree to abide by the Neighborhood Mermaid’s food intake policy for swim lessons. Agree Disagree Weather Policy * We do swim in light to moderate rain, as long as conditions remain safe. However, in the event of lightning or thunder, lessons will be paused or canceled immediately. We follow the national safety standard of waiting 20-30 minutes after the last thunderclap or lightning strike before returning to the pool. Weather-related cancellations are made at the instructor’s discretion with safety as the top priority. While make-up lessons and reschedules are not guaranteed, the instructor will do everything possible to accommodate and make up the missed time. If a lesson is interrupted or cut short after it begins due to weather, the remaining time will be added to a future lesson whenever possible. Agree Disagree Photo/Video Release * I grant permission for The Neighborhood Mermaid to take photographs or videos of my child during swim lessons for promotional purposes. I understand that no identifying personal information will be used.
